We are looking for Rehabilitation Services Coordinators (21-37 hours/week) to join us at Great Western Court in Gloucester. We provide 24-hour care for planned, unplanned and emergency stays. You’ll be part of a multi-disciplinary team helping individuals on their rehabilitation journeys, whether they’ve been discharged from the hospital or admitted from the community.
Great Western Court is conveniently located in central Gloucester, offering free parking and easy access by public transport. It's just a 13-minute walk (0.6 miles) from Gloucester train station and bus station. You can also hop on bus routes 1, 2, or 2a, which stop a short 5-minute walk away.
About the Role:
- Lead shifts and coordinate rehabilitation programs for individuals
- Safely administer, prompt, or assist with medication management.
- Provide essential mobility support and personal care, following the care plan and risk assessments
- Collaborate with the therapy team and other professionals to create and update care plans.
- Maintain accurate written and electronic records
- Facilitate smooth admissions and discharges, promoting independence
- Create a positive, stimulating environment for our residents.

About You:
- Hold a Level 3 qualification in Health & Social Care (or equivalent qualification, or experience) or be willing to work towards it.
- Have experience of administering medication and supporting individuals by following therapy-led rehabilitation goals
- Practical, positive, and ready to work in a team while managing your workload.
- Ready to make a meaningful impact with your care.
- Flexible for shift work, including weekends and sleep in duties when required (Early shifts start from 8am and late shifts will end at 10pm). You will be eligible to claim for enhanced payments for working evenings, weekends and bank holidays
You might be working in a similar role such as Senior Support Worker, Senior HCA or Senior Carer.
