About the role:
 

The successful candidate will oversee the implementation of a new case recording system within our Adult Social Care In-House Services for a fixed term of 12 months.  The successful candidate will be able to demonstrate the ability to carry out the following:
 

  • Project Collaboration: Work closely with stakeholders and the project team to ensure the successful implementation of the solution.
  • Project Planning:  Develop along with the successful supplier and key stakeholders a implementation plan incorporating any change management requirements.
  • Stakeholder Management: Serve as the primary point of contact for the project, maintaining regular communication and building strong relationships with stakeholders to understand their unique needs, expectations and issues with any solution.
  • Benefits identification and realisation: Work with service leads, front-line staff and stakeholders to identify benefits and measures of success, documenting and monitoring implementation and usage of the solution.
  • Supplier Management: Manage all day-to-day communications with the chosen Supplier and their nominated partners to coordinate the successful implementation of the necessary software, hardware, and services to achieve positive project outcomes.
  • Risk Assessment and Mitigation: Identify potential project risks, develop mitigation strategies, and proactively address issues to minimise disruptions and delays.
  • Quality Assurance: Oversee thorough testing and quality control processes, ensuring that the implemented systems and services meet established standards and fulfil requirements.
  • Project Documentation: Ensure comprehensive project documentation, including project plans, risk and issues logs, status reports are created and maintained, to facilitate knowledge sharing and project transparency.
  • Change Management: Develop and execute change management strategies to facilitate a smooth transition to the new systems and processes, including coordinating user training and supporting system champions to implement the change.
  • Stakeholder Communication: Communicate project progress, status updates, and key milestones to stakeholders, keeping them informed throughout the implementation process. Act as point of escalation from end users and report regularly to project board on progress/issues.
  • Data Imputing: Oversee and ensure relevant paper records are transferred accurately and timely in-line with the project plan.

You will have background in project management and a proven track record in delivering change, ideally in a social care setting.

About us:


GCC owns and operate a range of services for adults aged over 18 years old, which it refers to as its ‘In-House Services’. This includes:

  • Day services offering a range of activities in the building and in the community. This service supports adults with Physical disabilities and mild to moderate dementia, as well as Learning disabilities.
  • Reablement (at our Great Western Court facility) which provides a bed-based reablement service.  People accessing this service have no acute medical needs but do have specific person-centred physiotherapy and occupational therapy led goals to assist in recovery.
  • Respite services providing care and emergency placements for adults with learning disabilities and/or physical disabilities. Respite Services offer a range of activities both within the buildings and outside in the community.

 

The council is in the process of procuring a solution which will move the above services from recording individuals records on a paper-based system to a new digital solution.
