About The Role
An opportunity has arisen for a position as a full time Outcome Coordinator, specifically within the APS Team, to 31st August 2026, within the Education Inclusion Service. The primary focus of this role is to facilitate access to an education which is appropriate to a child’s needs, especially those who need additional support – ensuring that schools and other professionals work together so children attend school, make progress, and have managed transfers to other settings should this be required to meet their individual needs.
About You
The role of Outcome Coordinator includes:
- Championing children’s rights to appropriate education and coordinating input from a range of professionals to facilitate this.
- Monitoring outcomes for children who are more likely to be vulnerable and using the outcomes to target support and intervention in education settings effectively.
- Promoting early identification of need and supporting colleagues and settings to develop appropriate holistic assessments and support plans for children with additional needs.
- Liaising with a range of practitioners to ensure that appropriate services are involved in supporting individual children in a holistic way.
This OC role has a particular focus on the children entering and on roll in commissioned places at the Alternative Provision schools, ensuring the LA’s statutory duties are fulfilled and providing support where needed.
