Gloucestershire County Council Adult Social Care is looking for the right candidate to fulfil an exciting new opportunity, working closely with our partners, based in the newly created discharge hub at Gloucestershire Royal Hospital.

 

Our aim is to achieve, through multi-disciplinary working, the most appropriate and timely discharge from an Acute Hospital to a Home Setting or to a Bedded Pathway for recovery, assessment, and rehabilitation.

 

You will champion the role of Adult Social Care, undertaking the lead role in the hospital setting for professional social work practice, ensuring that discharges follow the Home First approach – if not, why not?

 

About the role:

 

This key post will be responsible for leading our hospital based social care team, providing professional knowledge and support to frontline practitioners who will be working alongside health partners to achieve the best outcomes for people in Gloucestershire. As the senior presence on site, you will operate as the professional social work leader at the critical interface between health and social care. Responsible for providing strong, diplomatic and confident leadership and credible challenge to further develop the integration of partnership working, you will ensure that peoples’ needs are always the priority and that the tenets of Home First and Make the Difference remain the underpinning principles of the service philosophy. The role will be highly visible and demanding, and you will need extensive social care expertise and resilience to thrive in this fast-moving environment where priorities are constantly changing, and where every decision made needs to ensure that the council meets its statutory obligations as required by the Care Act 2014.

 

 

About you:

 

You will be a qualified social worker who is registered with Social Work England.

 

You will have proven experience in the leadership, management, and development of teams, along with a desire to progress these skills further.

 

You will have substantial experience working within adult social care, and in working with partner agencies, stakeholders, and other professionals at a senior level.

 

You will be confident and resilient and enjoy working in a fast-paced environment where rapid but considered decisions need to be made to ensure the movement of people to the most appropriate environment to facilitate their recovery.

 

You will have in-depth knowledge and understanding of the legal and policy framework in relation to adult social care.
